RN Case Manager: Discharge & Care Coordination

Ochsner Health

Ochsner Health is seeking a Registered Nurse to manage discharge planning and utilization review across the hospital stay. You will review pre-admission, admission, and continued stay needs, coordinating with the care team to ensure medically necessary care and appropriate discharge plans.
