UCHealth is seeking a Diabetes Educator to deliver evidence-based education and self-management support to patients and families within our Healthcare & Medical Services department. In this role, you will assess learning needs, develop individualized care and teaching plans, and provide one-on-one and group education on nutrition, medications, glucose monitoring, technology, and lifestyle. Working closely with endocrinology, primary care, and inpatient teams, you will coordinate care, document in the EMR, track outcomes, and contribute to quality-improvement initiatives.
