In this patient-centered role, you will provide evidence-based diabetes self-management education, develop individualized care plans, and support patients and families across inpatient and outpatient settings. Working closely with physicians, nurses, and care coordinators, you will deliver culturally sensitive education on medications, nutrition, technology, and lifestyle changes.
