Avenue 360 Health and Wellness in Houston, TX seeks a Housing Case Manager to provide permanent supportive housing program case management and coordinated services. You will work with participants, property partners, and community resources to support housing stability and prevent homelessness. Responsibilities include eligibility assessment, enrollment, individualized service planning, and data management in HMIS.
