Key Responsibilities Educate patients, families, caregivers, and healthcare team members using appropriate teaching methods. Follow and adjust the therapist’s plan of care, documenting all communications and treatment modifications. Utilize standardized outcome measures to reflect functional performance accurately. Collaborate with the healthcare team, providing patient and family education, and communicate changes in patient status and discharge needs. Complete documentation adhering to regulatory and organizational policies. Qualifications & Skills Associate's Degree in Physical Therapy Assistance. Licensed as a Physical Therapist Assistant in Illinois. Basic Life Support (BLS) certification within 30 days. Completion of continuing education on acute stroke care and total joint replacement standards.
