YOUR CONTRIBUTION:

The Advanced Practice Provider (APP) is a vital member of our healthcare team that provides comprehensive medical care to patients living with HIV and at risk for HIV. The Advanced Practice Provider provides management of primary care health and wellness, urgent care assessments and the treatment of co-morbid conditions in an adult population. In this role, you will work collaboratively with an interdisciplinary health care team in addressing both the medical and social needs of our patients. This role requires strong clinical skills, empathy, and a commitment to delivering high-quality, patient-centered care to a diverse patient population.

Essential Job Duties * Perform comprehensive histories and physical examinations, order and interpret diagnostic tests, and develop appropriate assessments and ongoing evaluations of patients living with HIV/AIDS and at risk for HIV. * Develop and implement individualized pharmaceutical and non-pharmaceutical treatment plans both independently and in collaboration with the supervising physician and other healthcare professionals. * Monitor patients' progress, adjust treatment plans as needed, and address any complications or side effects. * Provide counseling and education to patients and their families regarding HIV/AIDS management, treatment adherence, and prevention strategies. * Collaborate with specialists, pharmacists, case managers, and other healthcare providers to provide integrated and comprehensive patient care. * Maintain accurate and up-to-date electronic health records, including documentation of patient encounters, treatment plans, and outcomes. * Participate in quality improvement initiatives, research projects, and continuing education activities to enhance clinical practice and patient outcomes. * This job description is not all-inclusive and outlines the main responsibilities and requirements of the role. KNOWLEDGE REQUIRED:

Required and preferred knowledge and experiences to succeed. * Master’s degree from an accredited Nurse Practitioner or Physician Assistant program. * Current state licensure (and any adjacent states as needed) and certification as an Advanced Practice Provider (NP or PA).

• Valid DEA license or the ability to obtain DEA licensure.
• Strong clinical assessment and diagnostic skills.
• Excellent communication and interpersonal skills, with the ability to

establish rapport with patients from diverse backgrounds.

• Ability to work effectively as part of a multidisciplinary team.
• Commitment to providing integrated and patient-centered care with compassion

and respect. * Proficiency in electronic health record (EHR) systems and other relevant technology. * Minimum of 2 years’ experience with primary care delivery in a clinical setting preferred. * Experience working in HIV/AIDS care preferred but not required. Salary: $120,000-$135,000/annually (Depending on Experience) The final salary will be determined based on factors such as market data, internal equity, candidate experience and qualifications. Please note that it is uncommon for an individual to be hired at or near the top of the range.
