Office Hours: 24 hours per week
Office Pay Rate: $30.00 per hourField/Direct Support Pay Rate: $21.00 per hour

Position Overview

Vision Life is seeking an experienced, compassionate, and highly organized Client Case Coordinator to join our growing Supported Living Services (SLS) team.

This is a hybrid office and field position. Candidates must understand that field days are a mandatory and essential part of this role. This is not an office-only Case Coordinator position.

IMPORTANT – SCHEDULE, FIELD REQUIREMENTS & PAY

The Client Case Coordinator is required to work both in the office and in the field.

• 3 office days per week / 24 office hours
• Office/Case Coordinator hours: $30.00 per hour
• Field/Direct Support hours: $21.00 per hour
• Field days are MANDATORY
• Coordinators must conduct required client house visits and field monitoring
• Must be available to travel to assigned client homes throughout Pasadena and surrounding Los Angeles County areas
• Must be willing to provide direct shift coverage when required
• Must be flexible to respond to client, staffing, and program needs in the field
• Mileage reimbursement is provided in accordance with company policy

Key Responsibilities

Under the direction of the SLS Program Supervisor and Program Director:

• Coordinate client services and staff scheduling
• Ensure authorized service hours are properly implemented
• Conduct intake assessments, screenings, and referrals
• Independently prepare IPP and ISP reports
• Manage assigned client caseloads
• Mentor, support, and oversee Direct Support Professionals (DSPs)
• Complete monthly, quarterly, and annual documentation
• Conduct mandatory client house visits and field monitoring
• Review client files, in-home binders, medication/service documentation, and required records
• Coordinate with Regional Centers, families, vendors, behavioral teams, and outside professionals
• Address client and staffing concerns
• Provide crisis intervention and behavioral support when necessary
• Provide direct client/shift coverage when required
• Participate in on-call responsibilities in accordance with company policy
• Ensure services remain person-centered, compliant, and focused on client independence

Qualifications

• High school diploma/GED with relevant experience required
• Associate’s or Bachelor’s degree in Human Services, Psychology, Social Work, or related field preferred
• SLS/ILS case management experience strongly preferred
• Experience working with individuals with developmental, intellectual, and/or behavioral disabilities
• Experience with California Regional Centers and DDS protocols preferred
• Experience independently completing IPP/ISP reports preferred
• Strong organizational, communication, and critical-thinking skills
• Ability to independently manage multiple clients and priorities
• Valid California Driver’s License
• Reliable personal transportation
• Current auto insurance and vehicle registration
• DOJ/FBI fingerprint clearance
• CPR/First Aid certification preferred
• CPI certification is a plus
• Must be able and willing to complete mandatory field days and direct support coverage

Schedule & Compensation

Part-Time

Office Component

• 3 office days per week
• 24 office hours per week
• $30.00/hour

Mandatory Field Component

• Required field days in addition to office responsibilities
• Includes client house visits, field monitoring, community-based support, and shift coverage
• $21.00/hour
• Field schedule will be based on client and program needs

This position requires both office and field work. Acceptance of the position includes acknowledgment that field responsibilities are mandatory.
