What You'll Do

• Assess and evaluate patients' physical functioning and wound care needs
• Develop and implement individualized care plans based on standardized assessments and evidence-based practices
• Administer procedures including manual techniques, therapeutic exercises, modalities, and use of assistive devices
• Monitor patient progress and modify treatments accordingly, documenting all services provided
• Provide education, counseling, and support to patients, families, and caregivers
• Communicate regularly with the healthcare team regarding patient status and treatment adjustments
• Complete discharge summaries and maintain accurate electronic health records
• Participate in meetings, trainings, and continuing education as required
• Supervise and support physical therapist assistants and rehabilitation aides as needed
• Adhere to state practice acts, ethical standards, and professional guidelines

What You Need

• Valid Physical Therapy license in Oklahoma or eligibility to practice
• Minimum of 2 years of relevant clinical experience in outpatient or LTC settings
• Strong assessment and treatment planning skills
• Excellent communication and teamwork abilities
• Ability to work independently and adhere to regulatory standards
