Job Type Full‑time Position Summary The Health and Wellness Director (LPN/LVN or RN) is an integral member of the management team and works closely with the Executive Director to ensure the health and wellness of our residents. They perform initial and ongoing assessments of residents' physical and psychosocial needs, coordinate with other departments to assure the highest quality of care, provide training, supervision, and coaching of the clinical team, and help build a positive atmosphere within the community. They are responsible for day‑to‑day clinical operations, ensuring compliance with all applicable federal, state, and local laws and company policies. Key Responsibilities Foster a resident‑centered approach to planning and delivering services, supporting residents’ rights to participate in decision‑making and treating them with dignity and respect. Assess residents’ health, functional and psychosocial status, initiate individualized service plans, and proactively manage care and services to maximize their opportunity to remain in assisted living or memory care. Establish and maintain the medication program. Oversee all hiring, promotions, disciplinary actions, and termination of licensed nursing staff (LPN/LVN), Med Techs/CMA/CNAs, and all care staff. Provide hands‑on training, supervision, and staff development. Communicate regularly with residents’ families regarding health care needs. Maintain current departmental policies, procedures, and licenses, following company, federal, state, and local requirements. Ensure resident records and charting are current and up‑to‑date. Participate in the management team’s marketing efforts by representing the community in a positive manner to outside agencies. Requirements

CANDIDATE QUALIFICATIONS

Education and Certifications Active LVN/LPN or RN nursing license in good standing. Associate degree or higher in nursing preferred, or equivalent education and experience; additional coursework in management is an asset. Current state‑required certifications, such as First Aid, CPR, and any other applicable credentials. Experience, Competencies, and Skills At least four (4) years of experience in geriatric nursing, including medication administration, delivery systems, and pharmacies. At least two (2) years in a supervisory/management role; experience with budgets, staff development, training, and scheduling. Experience in Memory Care preferred. Outstanding organizational skills. Proficient computer literacy and good knowledge of the Microsoft suite. Ability to work a flexible schedule, including some weekends, holidays, and overtime, with capacity to be on‑call 24/7.
