Key Responsibilities\n

• Educate clients and their families on community resources and support groups that enhance their social engagement and improve their overall quality of life.

• Maintain detailed and accurate documentation of client assessments, care plans, interactions, and progress notes to ensure compliance and continuity of care.

Compensation and Benefits\n
