About the Position

Job Title: Registered Nurse Profession: Nursing Specialty: Home Health Duration: 13 weeks Shift: Day Hours per Shift: 8 Experience: Minimum 1 full year of Adult Home Health Experience or 1 full year of Hospice experience depending on the specialty License: Must have active nursing license Certifications: CPR, BLS Must-Have: Must have recent specialty experience. Must be proficient in OASIS and have used it within the last 6 months, or complete a retraining course during compliance. Must be proficient in HomeCare HomeBase EMR and have used it within the last 6 months, or complete a retraining course during compliance. Must be willing to travel up to 75 miles from the assigned care center. Must have reliable transportation, a current driver's license, and agency-required liability insurance. Description: Provides coordinated and directed home nursing care according to physician's orders. Care is based on agency policies and procedures through the competent application of the nursing process. Responsible to the Clinical Manager. Directly supervises and evaluates Home Health Aides and Licensed Practical Nurses/Licensed Vocational Nurses. Home visits are required along with periodic office meetings. The average number of patients per day is 5-8. Contractors will be assigned between 5-10 patients per day, equating to approximately 6-8 points. The contractor must accept every patient on their schedule, regardless of distance from prior appointments. All documentation must be completed within 24 hours of the appointment. Delayed and incomplete documentation that impacts billing and follow-up patient care will result in a cancellation. No guaranteed hours are provided with this contract. Weekly productivity expectations are 30 points. Travel could amount to approximately 200 miles per week, depending on patient needs. The territory mileage radius requires flexibility for assignments based on patient needs.
