Position Overview

The Director of Patient Care Services is responsible for the planning, coordination, supervision, oversight, and evaluation of home health patient care services in accordance with agency policies, California home health requirements, Medicare Conditions of Participation, accreditation standards, and applicable federal, state, and local laws.

The DPCS works under the direction of the Administrator and collaborates with clinical, administrative, quality, compliance, and field staff to promote safe, effective, patient-centered care. The DPCS may also serve as the Clinical Manager or Alternate Clinical Manager if formally designated by the agency and if all applicable qualification requirements are met.

Essential Duties And Responsibilities

The Director of Patient Care Services is responsible for the following essential functions, with or without reasonable accommodation:

• Directs, supervises, and evaluates professional and paraprofessional personnel providing patient care services.

• Assists with planning, coordinating, leading, controlling, and evaluating home health services.

• Ensures that patient care services are delivered in accordance with physician or allowed practitioner orders, individualized plans of care, agency policy, and applicable regulatory requirements.

• Participates with the Administrator and leadership team in business planning, strategic goals, resource allocation, staffing plans, and annual budgeting.

• Assists in developing, implementing, and updating agency clinical policies, procedures, standards, and job descriptions.

• Assesses whether staffing levels are adequate and appropriate to meet patient care needs.

• Consults with supervisors and staff regarding patient care, special programs, service issues, and clinical operations.

• Evaluates clinical programs, patient care services, field personnel, and service delivery systems.

• Develops and supports standards that promote safe, effective, high-quality services for patients and families.

• Establishes and maintains effective channels of communication among agency leadership, clinical staff, administrative staff, patients, caregivers, and referral sources.

• Keeps clinical personnel informed of agency developments, patient care needs, regulatory expectations, and current clinical practices.

• Supports orientation, in-service education, continuing education, staff development, and competency-related activities.

• Oversees implementation and evaluation of patient care services, including compliance with licensure, certification, accreditation, quality, and documentation requirements.

• Collaborates with other departments and leadership personnel as appropriate based on the agency’s organizational structure.

• Assists with agency evaluation activities, quality assurance, performance improvement, and corrective action planning.

• Participates in development and maintenance of an effective patient care documentation system, including review of service delivery, utilization, quality indicators, and staffing-related data.

• Consults with physicians, allowed practitioners, and other healthcare providers regarding patient care services.

• Assists in evaluating community needs and developing programs or services to address identified needs.

• Participates in selected conferences, workshops, professional meetings, and educational activities to maintain knowledge of home health trends and regulatory expectations.

• Assists with lawful marketing, referral-source communication, and community relations, consistent with patient-choice, privacy, anti-kickback, and agency compliance policies.

• Participates in interviews and selection of home health personnel in accordance with equal employment opportunity, fair hiring, and agency policies.

• Maintains professional conduct and uses agency resources responsibly.

Clinical Manager Duties, If Designated

If formally designated as Clinical Manager or Alternate Clinical Manager, the DPCS may also be responsible for:

• Making patient and personnel assignments.

• Coordinating patient care.

• Coordinating referrals.

• Assuring that patient needs are continually assessed.

• Assuring the development, implementation, review, and updates of individualized plans of care.

• Performing any additional Clinical Manager duties required by Medicare Conditions of Participation, California requirements, accreditation standards, and agency policy.

Staff Development Responsibilities

The DPCS supports staff development by:

• Using the skills and experience of nursing and paraprofessional personnel in orientation and education programs.

• Identifying leadership potential and supporting appropriate training and development.

• Encouraging personnel to pursue continuing education, professional growth, and participation in relevant professional organizations.

• Supporting competency, training, and performance improvement activities consistent with agency policy and regulatory requirements.

Job Conditions

The employee must be able to perform the essential functions of the position, with or without reasonable accommodation. The agency will engage in the interactive process and provide reasonable accommodation where required by law.

The position may require prolonged periods of sitting, standing, walking, computer use, telephone or video communication, reading and reviewing records, preparing documents, attending meetings, and occasional lifting or carrying of office or clinical materials. Travel may be required for agency business, patient-care oversight, meetings, trainings, seminars, conferences, or community activities.

The position may involve occupational exposure to bloodborne pathogens, infectious materials, or other health-related risks. The employee must comply with agency infection-control, PPE, exposure-control, and workplace safety policies.

Qualifications

The Director of Patient Care Services must meet the following qualifications:

• Current, active California Registered Nurse license in good standing.

• A baccalaureate or higher degree in nursing or another health-related field, plus at least three years of experience within the last five years in a home health agency, primary care clinic, or health facility, including at least one year in a supervisory or administrative capacity; or

• If the Registered Nurse does not hold a baccalaureate degree, at least four years of experience within the last five years in a home health agency, primary care clinic, or health facility, including at least one year in a supervisory or administrative capacity.

• Sufficient clinical knowledge and decision-making experience for the patient populations served by the agency.

• Ability to contribute to quality assurance and performance improvement activities.

• Current CPR/BLS certification from an agency-approved provider.

• Ability to communicate effectively verbally and in writing.

• Ability to supervise, coach, and evaluate clinical and paraprofessional staff.

• Ability to maintain confidentiality and exercise sound professional judgment.

If designated as Clinical Manager or Alternate Clinical Manager, the employee must also meet applicable Clinical Manager qualification requirements, including required home care and supervisory experience under agency policy and applicable law.

Work Hours

The DPCS, or a qualified registered nurse designee where permitted by agency policy and applicable law, must be available on the premises or immediately accessible by telecommunications during operating hours when patients are receiving services.
