Key Responsibilities\n

• Conduct thorough psychosocial assessments to identify patients' mental health needs and develop individualized treatment plans.

• Diagnose and actively treat various mental and emotional disorders, adjusting treatment strategies based on patient progress and feedback.

• Implement and manage care plans that incorporate appropriate referrals to community resources, support groups, and additional mental health services as indicated.

• Work within a collaborative framework with patients' families, healthcare providers, and outside agencies to create comprehensive discharge plans and ensure continuity of care.

• Adhere to all standards set forth by federal, state, and local regulations, as well as align with the Mental Health Practice Act.

• Prioritize daily responsibilities by conducting regular evaluations of new patients and maintaining ongoing assessments for current cases.

• Provide psychosocial guidance, crisis intervention, and bereavement counseling to patients and their caregivers as necessary.

• Coordinate with multidisciplinary teams to resolve logistical challenges such as discharge delays or complex patient needs.

• Actively engage in advocacy on behalf of patients to facilitate effective communication among care teams and connect families with essential resources.

• Identify patients at risk for readmission or extended hospital stays, proposing strategic solutions to minimize these risks.

Qualifications\n

• A Master’s Degree in Social Work or a related field from an accredited institution, with verification of degree completion.
