Schedule & Coverage

• Monday–Friday, 8:00 AM – 5:00 PM
• Some weekend availability preferred:
• • Approximately 1 weekend per month
  • 1 on-call night per week
• Mileage reimbursement provided at $0.43 per mile
• Coverage areas include Escambia, Santa Rosa, Okaloosa, and Walton counties
• Primary service area for this position will be Okaloosa and Walton counties

Position Overview

We are seeking an experienced and compassionate Registered Nurse to provide skilled home health services to patients in the community. This role is responsible for conducting patient assessments, coordinating individualized care plans, and delivering high-quality clinical care to support positive patient outcomes. The ideal candidate has strong home health experience, excellent clinical judgment, and proficiency with OASIS documentation and Homecare Homebase systems.

Key Responsibilities

• Perform comprehensive assessments for high-risk patients in the home health setting
• Collaborate with physicians, primary care providers, and interdisciplinary teams to implement individualized care plans
• Continuously evaluate patient needs and adjust care plans to ensure appropriate and cost-effective care delivery
• Coordinate care transitions and communicate with physicians, social workers, discharge planners, and other healthcare professionals
• Arrange supportive services and resources to encourage compliance with treatment plans and improve patient outcomes
• Maintain detailed and accurate documentation of clinical, functional, and financial outcomes
• Identify opportunities for health promotion, illness prevention, and patient education
• Intervene promptly to minimize adverse patient outcomes and address clinical concerns effectively
• Advocate for patients while maintaining confidentiality and HIPAA compliance
• Ensure all patient care and documentation comply with company standards, professional practice guidelines, and state/federal regulations

Required Skills & Experience

• Active Florida RN license required; Compact RN license accepted
• Home health experience required
• OASIS and HCHB (Homecare Homebase) experience required
• Experience with:
• • Wound care
  • PICC line and IV care
  • Lab specimen collection
  • Drain care
  • Ostomy care
  • Post-surgical care preferred
• Strong assessment, critical thinking, and communication skills
• Ability to work independently and manage a patient caseload effectively

Qualifications

• Associate’s or Bachelor’s degree in Nursing or related field required
• Current, unrestricted Registered Nurse license
• Competency in Homecare Homebase/PointCare and OASIS documentation required
• Ability to analyze complex medical information and make sound clinical decisions
• Excellent organizational, documentation, and patient advocacy skills
