Description

• Conduct patient evaluations in inpatient and outpatient contexts and craft personalized treatment plans
• Carry out physical therapy interventions targeted at improving mobility, strength, balance, and overall function
• Partner with nurses, physicians, case managers, and other rehabilitation team members to enhance patient outcomes
• Educate patients and their families on therapeutic objectives, discharge planning, and at-home exercise regimens
• Document patient evaluations, treatments, and progress in accordance with facility and regulatory requirements
• Engage in care team rounds and interdisciplinary meetings to promote coordinated patient care

Education

• Doctorate in Physical Therapy (DPT) or an equivalent degree from an accredited institution

Certifications

• State licensure as a Physical Therapist
• CPR certification preferred
• Additional certifications in specialties (e.g., neurological, geriatric) are beneficial but not mandatory

Skills

• Prior experience in a similar role is strongly preferred
• Ability to serve patients with complex medical and mobility challenges
• Excellent communication and team collaboration abilities
