The Geriatrics & Extended Care Service at our healthcare system is on the lookout for a skilled and dynamic Board Certified Attending Physician in Geriatrics for the role of Medical Director at the Community Living Center. This position provides an opportunity to serve our veterans while enjoying an excellent work-life balance. The Community Living Center (CLC) offers Nursing Home-level care to veterans requiring short-term rehabilitation, skilled nursing, long-term care, dementia care, or hospice services.
Key ResponsibilitiesThe Medical Director of the Community Living Center is tasked with delivering patient care directly or overseeing CLC-assigned providers and medical trainees in both inpatient and outpatient environments. Responsibilities include:
- Providing care to adults with both acute and chronic medical conditions across a range of CLC services, including skilled nursing, long-term care, hospice/palliative care, pain management, and rehabilitation.
- Evaluating and managing adults with single or multiple disorders across various subspecialties, with a strong emphasis on geriatric medicine.
- Collaborating effectively within interdisciplinary teams to ensure comprehensive patient care.
The Physician in the Community Living Center is accountable for the overall quality of medical care provided to the veteran population. This role serves as the primary point of accountability in Geriatrics and Extended Care, ensuring the efficient use of resources to treat the assigned populations.
Clinical DutiesThe Physician will have the following direct clinical responsibilities:
- Reviewing patient histories, examinations, assessments, and care plans for CLC residents, coordinating evaluations, diagnostic testing, clinical follow-ups, and care coordination.
- Managing the day-to-day operations of the Community Living Center team in caring for veterans.
- Fostering an environment of efficiency and accuracy in the timely submission of completed documentation.
- Collaborating with Nurse Practitioners, Physician Assistants, ancillary healthcare providers, and various clinical specialties to facilitate hospital admissions, post-discharge care, and specialty consultations as necessary.
- Participating in the screening of veterans and completing consults for the CLC.
- Being part of the on-call schedule for telephonic coverage for urgent veteran needs in the CLC after administrative hours.
- Engaging in interdisciplinary team meetings.
- Taking on additional clinical duties as assigned by the Associate Chief of Staff for Geriatrics & Extended Care.
Our organization offers a comprehensive total rewards package, including:
- Recruitment Incentive: Sign-on bonus available.
- Salary: Competitive pay, annual performance bonuses, and regular salary increases.

- Retirement: Traditional federal pension (5 years vesting) and federal 401K with up to 5% contributions from the organization.
- Insurance: Federal health, vision, dental, term life, and long-term care insurance (many federal programs can be carried into retirement).
- Licensure: One full and unrestricted license from any U.S. state or territory.
- CME: Possible reimbursement of up to $1,000 per year (must be full-time with board certification).
- Malpractice: Free liability protection with tail coverage provided.
- Contract: No Physician Employment Contract and no significant restrictions on moonlighting.
- Education Debt Reduction Program (EDRP): Eligible participants may receive up to $200,000 towards qualified loans over a 5-year period (not exceeding $40,000 per year). EDRP is a reimbursement program requiring proof of payment to lenders for reimbursement.
Ideal candidates will possess:
