Collaborate with a multidisciplinary team to ensure comprehensive care. Educate patients and their families about health management and care options. Accurately document all aspects of patient care. Triage patient needs and coordinate appropriate services. Facilitate safe transitions of care for patients. Assist individuals in maintaining their independence and enhancing their quality of life at home. Qualifications and Requirements Current Registered Nurse license in the state of Colorado. Experience in home health care or a similar setting is preferred. Strong communication and interpersonal skills. Ability to work collaboratively within a team environment.
