The Role As a Clinical Pharmacist supporting Medicine and Cardiology services during evening shifts , you will be a key clinical resource for providers, nurses, and patients across the inpatient setting. Your work directly supports safe,evidence basedmedicationuseand continuity of care during a pivotal time of day. In this role, you will Provide comprehensive clinical pharmacy services to adult medicine and cardiology patients in an acute care academic setting Participate in interdisciplinary collaboration with physicians, advanced practice providers, nursing, and care coordination teams Optimize pharmacotherapy for complex cardiovascular and medical conditions , includinghigh riskmedication regimens Review and verify medication orders with a strong focus on safety, efficacy, andguideline directedtherapy Serve as a clinical consultant during evening hours, supporting therapeuticdecision makingand urgent clinical needs Contribute to transitions of care, medication reconciliation, andpatient specificeducation as appropriate What an average evening looks like Youstep into a highly collaborative environment where pharmacists are recognized as essential clinicians. MINIMUM REQUIREMENTS Education: Bachelor's in Pharmacy or Pharm. D Degree required. Experience: PGY-1 residency required or equivalent hospital experience. (3.years). PGY-2 residency is preferred for specialty areas. Licensure: Commonwealth of Virginia State Board of Pharmacy. License required. American Heart Association (AHA) Health Care Provider. BLS certification required PHYSICAL DEMANDS Job requires sitting for prolonged periods; Repetitive motion: typing.
