Key Responsibilities\n

• Conduct thorough psychosocial assessments to identify clients' mental health concerns and resources, ensuring a holistic understanding of their emotional and psychological wellbeing.

• Provide tailored individual, family, and group therapy sessions utilizing evidence-based therapeutic modalities such as cognitive-behavioral therapy, solution-focused therapy, and trauma-informed care.

• Develop and implement individualized treatment plans that include specific goals and interventions, regularly monitoring progress and adjusting strategies as needed.

• Collaborate closely with a multidisciplinary team, including psychiatrists, psychologists, and medical professionals, to coordinate care and enhance treatment efficacy.

• Facilitate crisis intervention, perform risk assessments, and establish safety plans to address urgent care needs effectively.

• Maintain meticulous documentation in accordance with ethical and legal standards, ensuring accurate client records and treatment histories.

• Lead group therapy sessions, psychoeducational classes, and support groups that foster community and resilience among clients and their families.

Patient Population / Client Focus\n

In this role, you will primarily work with patients receiving hospice care who may be experiencing mental health challenges related to anxiety, depression, grief, and trauma. Our goal is to support clients' emotional needs during this critical time, equipping them and their families with coping strategies and therapeutic support to navigate the complexities associated with end-of-life decisions.

Qualifications\n

• A Master’s degree in Social Work from an accredited program.

• An active state licensure as a Licensed Clinical Social Worker (LCSW).

• Strong interpersonal and communication skills, enabling effective collaboration and rapport-building with clients and team members.

• Demonstrable knowledge of evidence-based treatment approaches and therapeutic modalities relevant to mental health care.

Compensation and Benefits\n
