We currently provide home health care to patients in Santa Clara county, San Mateo County and San Benito county, and are expanding into Alameda, Contra Costa & Solano counties. Ascension Healthcare has an immediate need for a Home Health Physical Therapist in Santa Clara county, Sant Mateo County, San Benito County, Alameda County, Contra Costa County and Solano County. IF YOU WANT TO BE PART OF A DYNAMIC TEAM THAT OFFERS: Employee centered leadership who prioritized the safety, well-being and development of our staff Open, supportive culture that fosters collaboration, trust and respect Collaborative team environment dedicated to providing the highest level of care Culture of education, growth and celebrating the wins of patient satisfaction and quality improvement Highly organized office team dedicated to supporting our field clinicians Compensation - DOE with Oasis Home Health Duties: Complete OASIS start of care, evaluations, recertification, and discharge assessments Evaluate and assess patients' physical conditions and develop individualized treatment plans Provide hands‑on therapy, including exercises, stretches, and manual techniques Educate patients on proper techniques for exercises and activities to improve their condition Monitor and document patients' progress and adjust treatment plans as necessary Collaborate with other healthcare professionals to coordinate patient care Stay up‑to‑date with the latest research and advancements in physical therapy Experience: Bachelor's, Master's or Doctoratedegree in Physical Therapy Valid state license to practice as a Physical Therapist Strong knowledge of anatomy, physiology, and medical terminology Experience in a Home health is preferred. hospital or acute care setting is a plus Specialized training or experience in geriatric is a plus Familiar with Wellsky/Kinnser EMR documentation software is a plus As a Physical Therapist, you will play a crucial role in helping patients recover from injuries, manage chronic conditions, and improve their overall physical well‑being. You will work closely with patients to develop personalized treatment plans and provide hands‑on therapy to help them regain strength, mobility, and function. In this role, you will need a strong understanding of anatomy, physiology, and medical terminology to effectively assess patients' conditions and develop appropriate treatment plans. Experience in a hospital or acute care setting is preferred as it will allow you to work with a diverse range of patients with varying needs. Your ability to provide compassionate patient care, excellent communication skills, and attention to detail will be essential in helping patients achieve their goals. Additionally, specialized training or experience in areas such as trigger point therapy or pediatrics can be beneficial.
