Key Responsibilities\n

• Conduct thorough assessments of clients to identify their counseling needs and establish tailored treatment plans.

• Utilize DSM-5 criteria to assess clients for substance use disorders, determining the appropriate level of care according to ASAM guidelines.

• Develop and implement comprehensive treatment plans focused on substance abuse counseling, collaborating with multidisciplinary teams.

• Create detailed client discharge plans to facilitate seamless transitions to various levels of care when needed.

• Document all counseling sessions, assessments, and follow-ups meticulously in clients' clinical records to ensure continuity of care.

• Maintain a manageable caseload, performing regular case management duties and utilizing review updates to monitor client progress.

• Coordinate support services and resources for clients and their families to optimize their treatment experience.

• Engage with external referral sources such as legal representatives, educational institutions, and employers to provide holistic support.

• Utilize crisis intervention techniques as needed, working with a multi-disciplinary team to address urgent client situations effectively.
