Education Requirements

• MD or DO from a recognized medical institution.
• Completion of an accredited residency program in Family Medicine.

Certifications

• Board Certification in Family Medicine.
• Eligible for (or currently possess) an active Washington state medical license.
• BLS and ACLS certifications are desirable.
