As part of our dedicated team, you will provide direct patient care while reporting to the Director or Manager. Collaboration is essential to our success, and nurses work closely with physicians, therapists, case managers, and fellow caregivers to develop and implement individualized care plans that support optimal patient outcomes. We are committed to supporting one another, celebrating successes together, and creating an environment that promotes professional growth, continuous learning, and excellence in rehabilitation care.How you'll contributeA Registered Nurse (RN) who excels in this role:Accurately performs patient assessments and identifies patient needsIdentifies and initiates appropriate nursing interventionsProvides care appropriate to condition and age of the patientPerforms timely and appropriate documentation relating to medical necessity in the medical recordResponsible for completion and revision of the Interdisciplinary Care Plan for each patientPerforms timely and accurate QI assessmentsWhat we're looking for:Applicants should have a current state RN license and possess a bachelor's degree from an accredited nursing school.
