Description

• Conduct evaluations in both inpatient and outpatient settings while crafting personalized treatment plans
• Deliver physical therapy treatments targeting mobility, strength, balance, and overall function
• Work in collaboration with nurses, physicians, case managers, and other rehabilitation experts to improve patient outcomes
• Guide patients and their families through therapeutic goals, discharge planning, and at-home exercises
• Maintain accurate documentation of evaluations, treatments, and patient progress according to facility and regulatory standards
• Engage in interdisciplinary rounds and care conferences to achieve coordinated patient care

Education

• A Doctorate in Physical Therapy (DPT) or equivalent from an accredited institution

Certifications

• Current state licensure as a Physical Therapist
• CPR certification preferred
• Specialized certifications (e.g., neurological, geriatric) are advantageous but not required

Skills

• Prior experience is highly preferred
