Pay Range: $18.5/hour. Employee benefits include, but are not limited to, health insurance (medical, dental, vision), 401(k) plan, and paid sick leave (depending on work location).

Key Responsibilities:

• Provide outreach to members via phone to support care plan next steps, community resources, and scheduling concerns.
• Serve as the front-line support for member and provider inquiries, explaining care plan procedures and protocols.
• Support member onboarding and day-to-day administrative duties, including sending welcome letters and educational materials.
• Document and maintain non-clinical member records to ensure compliance with state and regulatory requirements.
• Make referrals to address Social Determinants of Health (SDOH) needs and ensure members receive high-quality customer service.
• Meet a daily goal of contacting at least 30 members to schedule assessments and follow up within strict turnaround times.

Key Requirements and Technology Experience:

• 1 to 2 years of call center experience, preferably in a healthcare or managed care setting.
• High School Diploma or equivalent.
• Proficiency in Microsoft Office, including Outlook and Excel.
• Strong customer service skills and ability to handle high call volumes.
• Bilingual skills are a nice to have .
