Key Responsibilities

• Conduct comprehensive evaluations of patients, reviewing their medical history, performing specific assessments, and documenting results to inform treatment plans.
• Communicate evaluation findings and the proposed treatment plan to patients and their families, incorporating their input into goal-setting and care strategies.
• Implement and adapt treatment protocols based on evaluation outcomes and changes in patients' health status, ensuring a continuously evolving approach to care.
• Collaborate with a multidisciplinary team of healthcare professionals, recognizing shifts in patients' conditions that necessitate adjustments in their care plans.
• Manage discharge planning and ensure effective follow-up care, maintaining detailed and accurate documentation of all patient interactions and progress.

Patient Population / Client Focus

In this role, you will primarily work with adult patients recovering from surgery, injuries, and chronic conditions affecting mobility and daily functioning. Qualifications

• A Bachelor's degree in Physical Therapy from an accredited institution is required.
• Valid Physical Therapist License in Virginia or eligibility to obtain one.
• Current Basic Life Support (BLS) certification through the American Heart Association.

Preferred Experience
