Key Responsibilities\n

• Utilize your education, training, and hands-on experience to conduct independent psychosocial assessments and develop treatment plans tailored to individual patient needs.

• Formulate patient-centered treatment plans aimed at addressing psychological, emotional, and social challenges faced by patients.

• Employ evidence-based interventions—including solution-focused therapy, behavioral activation, and motivational interviewing—to effectively support patients through their journeys.

• Collaborate with an interdisciplinary team, including primary care providers, psychologists, and other mental health professionals, to deliver comprehensive and holistic care.

• Regularly monitor patient progress both virtually and in-person, tracking treatment outcomes to adapt care plans as needed.

• Assist in managing psychotropic medication adherence, providing education on medication side effects and monitoring overall treatment effectiveness under the direction of medical providers.

Qualifications\n
