Key Responsibilities\n

• Conduct thorough assessments of patients’ mental and emotional well-being to develop tailored treatment plans that address individual needs.

• Maintain accurate and detailed records of patient interactions and progress, adhering to HIPAA regulations and confidentiality standards.

• Collaborate closely with interdisciplinary teams, including physicians, nurses, and chaplains, to coordinate comprehensive care strategies.

Patient Population / Client Focus\n

Your primary focus will be on hospice patients and their families, who often experience complex emotional challenges. You will work with individuals facing end-of-life issues, providing them with the therapeutic support needed to navigate their unique experiences. This role is especially impactful, as you will help patients find peace and closure while supporting their loved ones through grief and loss.

Qualifications\n

• A Bachelor’s Degree in Social Work from an accredited institution.

• An active LMSW license in the state of New Jersey.

• Demonstrated proficiency in therapeutic modalities with a focus on trauma, anxiety, and depression.

• Excellent written and verbal communication skills, with the ability to build rapport and trust with clients and their families.

• A proven ability to work independently while contributing to a collaborative team environment.

Preferred Experience\n

We are looking for candidates who have two years of experience in a clinical setting, ideally with exposure to hospice or palliative care. Familiarity with telehealth platforms is highly advantageous, as is experience working within a multidisciplinary team integrated model.
