Key Responsibilities\n

As a Licensed Clinical Social Worker at Optimum Care Center, you will engage in the following key responsibilities:

• Conduct comprehensive clinical assessments to determine individual patient needs and develop personalized treatment plans that integrate evidence-based and solution-focused modalities.

• Utilize various therapeutic interventions such as cognitive-behavioral therapy (CBT) and dialectical behavior therapy (DBT) to address psychological distress and promote mental well-being.

• Maintain accurate, timely clinical documentation in compliance with HIPAA and state regulations, ensuring the confidentiality and safety of patient information.

• Collaborate closely with a multidisciplinary treatment team, including psychiatrists and chaplains, to coordinate holistic care and support for patients and their families.

• Participate in ongoing case consultations and peer reviews to uphold the highest standards of professional practice.

• Offer education and resources to patients and families regarding mental health issues, treatment options, and available community resources, facilitating informed decision-making.

• Stay up-to-date with ongoing developments in mental health practices and incorporate new knowledge into therapeutic strategies.

Your primary focus will be on working with patients and families in a hospice setting, facing complex emotional challenges during life-limiting illnesses. You will play a critical role in addressing mental health concerns related to grief, loss, fear, and anxiety, helping them navigate this difficult phase of life with dignity and support.

Qualifications\n

• A Master’s degree in Social Work from an accredited institution.

• A valid license to practice as a Licensed Clinical Social Worker (LCSW) in Pennsylvania.

• A minimum of 3 years of clinical experience providing psychotherapy services.
