The Cognitive Care Center is seeking a dedicated and licensed Clinical Social Worker to join our dynamic team. This full-time, remote position focuses on providing critical psychosocial support to seniors in our hospice facility located in Aurora, Illinois. Work Environment\n

In this remote role, you will enjoy flexible working hours, although interaction with clients may necessitate occasional visits to our hospice center or other community settings. You will typically work Monday through Friday, 8:00 AM to 4:30 PM, with the potential for evening or weekend hours based on client needs. Flexibility is essential to accommodate visits and ensure continuous care for our clients.

Key Responsibilities\n

• Conduct comprehensive assessments that evaluate the emotional, psychological, and social needs of seniors within our hospice care.

• Develop personalized care plans in collaboration with clients and their families, tailoring strategies that address specific goals, preferences, and values.

• Provide individual and family counseling sessions aimed at helping clients cope with life transitions, chronic illness, and end-of-life planning.

• Advocate for clients' rights, assisting them in accessing critical community resources, services, and support systems.

• Engage in regular communication with an interdisciplinary team, including healthcare professionals, to ensure holistic care that meets all facets of a client's well-being.

• Monitor and document client progress, adjusting care plans as necessary to promote continued growth and comfort.

• Educate clients and their families about the hospice process, available resources, and ways to enhance social engagement within the community.

• Facilitate family meetings to improve communication among caregivers, clients, and family members, ensuring everyone is informed and involved in the care process.

• Maintain accurate, timely documentation in compliance with regulatory standards and best practices in social work.

Patient Population / Client Focus\n

You will primarily work with seniors facing various end-of-life challenges, including managing chronic illnesses, navigating emotional distress, and preparing for hospice care. Your role is crucial in providing emotional support not just to clients but also their families, helping them cope with the complexities that arise during this critical phase of life.

Qualifications\n

• Must possess a Master’s degree in Social Work from an accredited institution.

• Valid Illinois state licensure as a Licensed Clinical Social Worker (LCSW) is required.

• A minimum of two years' experience in a social work role, particularly in a geriatric, hospice, or community-based setting.

• Strong understanding of social work principles, particularly as they pertain to aging populations.

• Exceptional assessment, counseling, and conflict-resolution skills.

Compensation and Benefits\n

The position offers a competitive annual salary ranging from USD 60,000 to 100,000, commensurate with experience and education.
