Community Health Navigator: Field Guide to Impactful Care

Reema Health, Inc.

Reema Health is seeking a Community Health Navigator to build trusted relationships with members and guide engagement in their healthcare. You will manage a caseload, use Reema's platform daily to reach members, schedule visits, and document progress. Roles are dynamic in a growing startup, with opportunities to grow and innovate as needs evolve.
