Case Manager

Wisconsin Community Services seeks a Case Manager for our Community Support Program in Milwaukee, WI. In this full-time role, you will provide trauma-informed, client-centered case management to adults with behavioral health and related needs, many of whom have justice involvement. Responsibilities include intake, assessment, person-centered service planning, coordination of behavioral health, housing, and employment services, and advocacy to promote recovery and community stability. You will collaborate with multidisciplinary teams, engage families and supports, respond to crises, and maintain accurate documentation, all within a mission-driven, inclusive, and equity-focused culture.
