Habilitation Specialist

Instructs and supports individuals in developing and/or enhancing life skills necessary to live more independently in home and community settings; and develops, implements, and modifies habilitation plans to help individuals achieve their valued outcomes.

Assist and support individuals in acquiring, retaining, and improving the self-help, socialization, and adaptive skills necessary to live successfully in home and/or community-based settings. Incumbents work directly with individuals to help them lead better lives; develop and implement person-centered plans for habilitation services; assist in setting goals; and monitor individuals' progress in achieving their valued outcomes.

Primary duties and responsibilities include, but not limited to:

• Participates as a member of the Interdisciplinary Team in the Life Plan process, along with the Individual, family/advocate, Care Manager and other service providers.
• Assesses individual's skill levels and identifies appropriate methods to teach/assist individuals in acquiring, improving, and maintaining life skills.
• Completes required habilitation documentation in accordance with agency and program requirements.
• Develops and updates Staff Action Plans (SAPs) in accordance with the Life Plan.
• Develops and updates Safeguard Summary Plans (SSPs) as needed.
• Train staff on the implementation of Staff Action Plan outcomes and documentation of individual's response to services.
• Monitor the implementation of Safeguard Summary Plan (SSP), Staff Action Plan (SAP).
• Review all documentation, assess status and progress of the Staff Action Plan (SAP) and Safeguard Summary Plan (SSP).
• Revise as needed to ensure that plans continue to reflect the individual's abilities and need for protective oversight and supports.
• Review Daily Service Recording monthly and generate Monthly Summary Notes based on the implementation of the Staff Action Plan.
• Participate in activities which support the program(s) and/or the unit's approach to providing optimum care for all individuals.
• Participate in the Life Plan review meetings, Team Meetings, ad hoc meetings, as needed, and take minutes when required.
• Participate in Programmatic Self-Assessment and Regulatory Plans of Corrective Action (POCA) development, implementation and monitoring as relevant to the QIDP role.
• Completes and maintains habilitation records and required documentation in accordance with agency standards.
• Participate with other Team members in a variety of individual, Team and continuous improvement meetings and forums as scheduled to promote comprehensive and individualized services.
• Must have the ability to adjust work hours to meet operational/supervisory needs including but not limited to Life Plan Meetings, trainings, and/or off shift visits.
• Complete additional tasks, as assigned, to meet departmental needs.

• Strong organizational and time-management skills.
• Excellent computer skills (Microsoft Suite, including Excel)
• Professional written and verbal communication approach.

This title is part of the New York Hiring for Emergency Limited Placement Statewide Program (NY HELPS). HELPS Program titles may be filled via a non-competitive appointment. This means that you do not need to take an exam to qualify, but you do need to meet the minimum qualifications of the title. At a future date (within one year of permanent appointment), employees hired under NY HELPS are expected to have their permanent non-competitive employment status converted to permanent competitive status. You will not have to take an exam to gain permanent competitive status.
