Field-Based Nurse Care Manager: In-Home Health

Humana

Humana is seeking a Field Care Manager Nurse 2 in Michigan to assess and guide members toward appropriate resources, performing home, community, and telephonic assessments. You will develop and coordinate care plans with an interdisciplinary team, support caregivers, and travel predominantly within Wayne/Macomb counties.
