The role includes both pre and post-transplant nursing coordinator responsibilities. Key responsibilities involve managing new referrals, pre-transplant evaluation, selection conference, and waiting list management. The post-transplant aspect includes providing longitudinal care for over 500 patients. Duties encompass lab monitoring, implementing immunosuppression dose changes, and addressing transplant-related comorbidities. The position is primarily office-based, with patient interactions occurring in an outpatient clinic three mornings per week. The role supports a team of three other Registered Nurses in the department. Candidates must be available to work four to five days a week. Case management experience is beneficial for effective care coordination. Additional skills include biohazard waste disposal, medication reconciliation, and patient teaching. The position does not require weekends, call, or holidays. The charting system used is EPIC.
