Travel RN Case Manager

We are seeking an experienced Travel RN Case Manager for a home health position in Brookings, OR. The role involves managing a caseload and providing care primarily within Brookings and neighboring northern Curry County.

Contract Details

Contract Length: 13 or 26 weeks
Schedule: Monday Friday, 8 AM 5 PM
Guaranteed Hours: 40 per week
Weekends/Holidays: Not anticipated

Responsibilities

Conduct start-of-care visits, recertifications, and discharges
Supervise and coordinate staff in home health settings
Manage approximately 27 patient visits per day, depending on visit type and travel time
Documentation time is included within the workday
Primarily responsible for home health, with no hospice coverage
Ensure effective care coordination for patients

Travel Requirements

The average travel distance will be approximately 60 miles one way, with mileage reimbursement provided.

Qualifications

Active Oregon RN license required
Previous experience in home health is required
Experience with OASIS is required
Proficiency in EMR systems, specifically Axxess
