Including the oversight of medical appointment, medication management, provide advocacy for treatment on behalf of the child, consultation and education to foster parents, birth parents and staff via in person contacts with the persons listed. May work evening and some weekends in order to complete tasks. May travel to high risk communities. Proficient in effectively responding to and managing crisis situations, providing immediate support, and following established crisis management protocols to ensure the safety and well‑being of clients or individuals served. Actively assist in identifying chronic physical, developmental, dental, and mental health conditions, promptly documenting such and providing prompt referral and collateral information as needed. Assist LCFS CWS and families in obtaining additional services necessary to educate and support caregivers in meeting the children's needs. Collect health information to determine/facilitate timely linkage to referrals and services needed. Work with healthcare professionals and specialists to ensure that children receive appropriate medical treatment and interventions. Maintain detailed and accurate health records, progress notes, and documentation in compliance with agency and regulatory requirements. Prepare reports on the health status and care plans for children in the program. Conduct Healthy child checkups once a quarter. Must spend on average one hour per month with every child in medical specialized foster care contracts within the designated region. Must attend well-child coordination (s) meetings (as indicated by supervisor if not hosting) Must conduct quarterly review of coordinated care including overall report of coordinated care and any recommendations. Collaborate with child welfare teams, case workers, and legal professionals to advocate for the best interests of children's health. Attend case conferences, court hearings, and meetings to provide expert recommendations and support. Knowledge and Ability Requirements: Advanced knowledge of pediatric nursing principles, healthcare practices, and child health assessments. Proficiency in delivering health education and guidance to children, families, and professionals. Excellent communication skills, both verbal and written, for effective interaction with children, caregivers, healthcare providers, and agencies. Ability to maintain and manage health records and documentation in compliance with regulations. Ability to provide support and healthcare in crisis situations. Education and Experience: Must hold a valid and current Registered Nurse license. Ideally possess a pediatric nursing specialization or relevant experience. Must have at least 2-3 years of progressive experience in pediatric nursing or a related role, with experience in child health assessments. Physical Demands and Other Requirements: To perform this job successfully, an individual must be able to perform the essential duties listed in the job description satisfactorily. While performing the duties of this job, the employee is regularly required to talk or hear. The employee is frequently required to sit. The employee is occasionally required to stand; walk; use hands to finger, handle or feel objects, or controls; reach with hands and arms. Capable of operating standard office equipment, including a computer and keyboard, calculator, typical business machines such as a fax machine and copier. The employee must occasionally lift and/or move up to 20-30 pounds. Specific vision abilities required by this job include close vision, distance vision and the ability to adjust focus. The employee is required to have valid driver’s license, auto insurance and must be able to safely operate a motor vehicle. The work environment characteristics include a noise level which is usually low to moderate. Informs appropriate management staff when incident occurs or a crisis situation. Maintains professional boundaries with clients/residents, staff, interns, volunteers, and vendors. Able to successfully share skills and concepts with others and works effectively with others under time and environmental pressures. Safety and Agency Resource Utilization: Demonstrates sound judgement and foresight in safety awareness and in daily work. Notifies, in a timely manner, the supervisory chain of any incidents or circumstances that need attention. Report all suspicions or allegations of abuse immediately to their supervisor. Ensures appropriate utilization of materials and resources and minimizes waste. Other Responsibilities: All other tasks and responsibilities as assigned.
