The candidate must reside within the following county(s) within Region 8: Bexar, Kerr, Bandera, Gillespie, Kendall, Comal, Guadalupe, Frio, La Salle, Atascosa, Wilson, Karnes, Victoria, De Witt, Lavaca, Calhoun, Jackson, Val Verde, Edwards, Real, Kinney, Uvalde, Medina, Maverick, Zavala and Dimmit County.
This position has a hybrid schedule and will be required to be on site occasionally.
PRIMARY FUNCTION/RESPONSIBILITY
The Texas Family First (TFF) Referral Coordinator is responsible for coordinating and managing all incoming Texas Family First referrals across assigned service regions. This position serves as the primary point of contact for referral staffing and coordination, ensuring referrals are reviewed thoroughly, staffed appropriately, and processed in compliance with Texas Family First policies, procedures, and contractual requirements.
Additionally, this role ensures timely staffing of cases, effective referral management, and alignment of client needs with available services. The position plays a critical role in supporting service delivery, improving client outcomes, and maintaining compliance with organizational and funding requirements.
The Referral Coordinator will communicate closely with DFPS staff, court personnel, service providers, subcontractors, and internal program staff to ensure timely referral processing and enrollment into court-ordered services. This role requires strong organizational skills, attention to detail, professional communication, critical thinking, and the ability to manage multiple priorities in a fast-paced environment.
ESSENTIAL DUTIES AND RESPONSIBILITIES
Referral Coordination & Staffing
- Serve as the primary point of contact for all incoming Texas Family First referrals.
- Answer and manage the designated referral staffing line and gather all necessary referral information.
- Conduct referral staffing reviews to determine appropriateness and eligibility based on Texas Family First policies and program requirements.
- Document referral staffing decisions accurately and timely within required systems and documentation processes.
- Communicate referral decisions, recommendations, and next steps to DFPS staff, court personnel, service providers, and internal staff.
- Ensure referrals are processed efficiently to support timely enrollment into court-ordered services.
- Match clients with appropriate case managers, programs, or providers based on expertise, caseload capacity, and client needs
- Facilitate case staffing meetings to review complex or high-risk cases
- Ensure equitable distribution of cases across staff and programs
- Monitor caseload levels and recommend adjustments as needed
Documentation & Case Coordination
- Track and maintain all referral-related documentation and records.
- Request and obtain missing documentation or records needed to complete referral processing.
- Ensure all referral files are complete, accurate, and compliant with contractual and organizational requirements.
- Maintain organized referral tracking systems and provide updates regarding referral status as needed.
- Add court dates, staffing meetings, and other required events to program calendars and tracking systems.
Communication & Collaboration
- Communicate referral information and case updates to the Parents as Teachers (PAT) Supervisor and contracted service providers.
- Collaborate with DFPS staff, supervisors, subcontractors, and community partners throughout the referral and enrollment process.
- Maintain ongoing communication with supervisors and DFPS staff until the family has been successfully enrolled into court-ordered services.
- Demonstrate professionalism and strong customer service when interacting with families, stakeholders, and community partners.
- Participate in meetings, staffing’s, and collaborative discussions related to Texas Family First services and referrals.
Program Support & Compliance
- Ensure referral processes remain compliant with Texas Family First policies, procedures, and contractual expectations.
- Support program operations by assisting with data collection, reporting, and referral tracking activities.
- Identify barriers or delays in referral processing and communicate concerns to leadership.
- Utilize problem-solving and critical thinking skills to support timely and effective service coordination.
- Maintain confidentiality and protect sensitive client information in accordance with agency and legal requirements.
Work Environment and Travel Requirements:
This position operates in a hybrid work environment, with the majority of duties performed remotely. Employees may be required to attend meetings, trainings, and other work-related activities at various locations within SJRC Texas's 28-county service area. Reasonable advance notice will be provided for required travel and in-person attendance. Regular access to reliable transportation is required.
QUALIFICATIONS
Required:
- Bachelor’s degree in Social Work, Human Services, Psychology, Criminal Justice, Public Administration, or related field preferred.
- Minimum of 2 years of experience in child welfare, family support services, case coordination, or related field.
- Knowledge of child welfare systems, family preservation services, and community-based programming.
- Strong organizational, communication, and documentation skills.
- Ability to manage multiple tasks, deadlines, and priorities simultaneously.
- Ability to maintain professionalism and confidentiality in high-pressure situations.
- Strong attention to detail and ability to navigate complex systems and processes.
- Proficient in Microsoft Office programs and electronic documentation systems.
Preferred:
- Experience working with DFPS and court systems.
- Knowledge of Texas Family First or prevention-focused child welfare programs.
- Experience coordinating referrals, staffings, or community-based services.
- Bilingual (English/Spanish) preferred.
Certifications/Licenses/Registrations:
- Valid Texas Driver’s License, good driving record, ability to provide own transportation and proof of current automobile insurance.
- CPR/First Aid Certification.
- Minimum age of 21 years per licensing standards.
PHYSICAL/MENTAL REQUIREMENTS AND INFORMATION
- Body mobility to stoop, kneel, bend, reach, walk and walk briskly in order to interact with and monitor children.
- Stamina to work long days and drive long distances.
- Moderate to heavy lifting up to 60 pounds.
- Must have the ability to communicate both verbally and in writing.
- Must have the ability to give and receive verbal and written instructions (with or without hearing aids).
- Must have fine vision, sustained vision, and peripheral vision (with or without glasses).
- Environment requires dexterity to remain calm and adapt to fast paced and emotional environments consisting of children with moderate to severe emotional and psychological needs.
- Home-like settings with varying moderate to high degrees of background noise.
- Light and ventilation are found in a typical home setting.
- Campuses are smoke-free, except on permitted campuses in designated areas.
- Possibility of exposure to blood borne pathogens exists if universal precautions are not followed (Hepatitis B inoculation available per request).
- Possible injury when lifting or moving if employee fails to use proper body mechanics, or if proper techniques are not used.
