Key Responsibilities:

• Perform home health risk assessments, averaging 45 minutes per visit, which may include annual health reviews or post-discharge assessments
• Assess prior and current medical history, chronic conditions, and medication use
• Educate members about their medical conditions and treatment regimens
• Identify care gaps and offer actionable recommendations to enhance health outcomes
• Document visits clearly and efficiently for Medicare Advantage compliance

Qualifications:

• Active Nurse Practitioner license in Virginia
• Master of Science in Nursing (MSN) from an accredited institution
• Board certified by AANP or ANCC
• Current CPR certification (BLS)
• At least six months of Nurse Practitioner experience preferred
• Strong interpersonal, assessment, and documentation skills
