Outpatient Addiction Counselor – QHP (LMHC/LMSW)Make a Difference in RecoveryAre you a QHP who is passionate about helping individuals and families affected by substance use?We are looking for a compassionate, dedicated, and experienced Outpatient Addiction Counselor to join our team. This is an opportunity to make a meaningful difference while working in a supportive, collaborative outpatient treatment environment.The ideal candidate will have experience providing substance use counseling and be comfortable providing both individual and group counseling, conducting assessments, developing treatment plans, coordinating services, and maintaining clinical documentation.What You'll DoProvide individual and group counseling to clients receiving outpatient substance use treatment.Conduct screenings, assessments, and ongoing clinical evaluations.Develop and update individualized treatment plans.Maintain a caseload of up to 35 clients.Provide counseling services to adolescent clients within the scope of credentials and applicable regulations.Coordinate care and make appropriate referrals to medical, mental health, social service, and community resources.Maintain accurate and timely documentation in the agency's EMR system.Collaborate with the clinical team regarding treatment planning, case coordination, and client progress.Participate in staff meetings, case conferences, trainings, and committees.Maintain a safe, respectful, and supportive treatment environment.Build positive professional relationships with clients, families, coworkers, referral sources, and community providers.Follow all agency policies, procedures, and applicable state and regulatory requirements.Participate in ongoing professional development and continuing education.Perform other duties as assigned by the Program Director.What We're Looking ForRequired:Qualified Health Professional License (QHP – LMHC / LMSW)Experience providing substance use counselingStrong communication, organizational, and documentation skillsAbility to work effectively as part of a multidisciplinary treatment teamPreferred:3+ years of counseling experience, preferably in an OASAS licensed outpatient substance use treatment programExperience working with individuals with co-occurring mental health and substance use disordersExperience with electronic medical records and Microsoft OfficeWhat We OfferWe value the people who provide care to our clients.
