Registered Nurse - Case Management - Hospital

Job Description

Job Summary:

Seeking an experienced Registered Nurse with at least one year of inpatient Case Management expertise to join a community-based hospital team. The role involves managing patient plans within the hospital setting, ensuring compliance, and coordinating care to promote positive outcomes.

Responsibilities:

• Evaluate patient needs and develop individualized discharge plans.
• Collaborate with healthcare teams to coordinate patient care and services.
• Document clinical interventions accurately using Cerner charting system.
• Maintain compliance with hospital protocols and safety standards.
• Manage a patient caseload within designated ratios, ensuring quality care.
• Adhere to scheduling requirements, including 4x10 hour day shifts from 8:00 to 16:30.

Qualifications:

• Hold a valid RN license, with pending license acceptance permitted.
• Demonstrate at least 1 year of inpatient Case Management experience.
• Possess current BLS certification.
• Be fully vaccinated against COVID-19, with medical or religious exemptions documented.
• Capable of working in a Level III Trauma Center with a Level I Teaching designation.
• Willing to work the Thurs-Sun schedule and comply with travel and RTO policies.
