Outpatient PharmacistThe Outpatient Pharmacist is responsible for maintaining the daily operations of the Outpatient Pharmacy to ensure accurate and timely service. Pharmacists are responsible for providing medications and other supplies as needed for adequate patient care according to professional standards and practices. Additionally, outpatient pharmacists evaluate drug therapies, counsel patients, immunize, and perform medication therapy management.Essential Duties and Responsibilities include:Directing the daily workflow of the Outpatient Pharmacy to ensure accurate and timely dispensing and/or delivery of medications and supplies.Reviewing prescription orders for appropriateness, including safety and efficacy.Performing prospective drug utilization reviews, assessing for proper dosage, use directions, drug interactions, contraindications, duplicate therapies, and other regimen-related considerations, and making interventions as needed.Performing medication filling, checking, and review.Managing pharmacotherapeutic services in accordance with institutional or state standards.Providing immunizations and other direct patient care as indicated, in accordance with institutional or state practice standards and/or protocols.Assigning support duties to technicians, interns, and students as needed to ensure completion of all tasks.Counseling patients and families regarding all aspects of medication use, including for hospital discharge patients as needed to support the Meds-to-Beds program (medication preparation and delivery).In conjunction with the pharmacy manager and buyer, assisting with inventory management and compliance with inventory processes.Providing oversight of the cash management system, ensuring accounting receives appropriate records of all transactions, and providing a daily cash report.Maintaining appropriate records for all medications and processes in the pharmacy, including controlled substances and other required documentation.Maintaining competency with pharmacy knowledge and techniques to ensure quality care for patients of all ages.Following established department policies, procedures, objectives, quality assurance programs, and safety, environmental, and infection control protocols.Demonstrating competency with pharmacy automation, medication distribution, and applicable technology.Contributing to departmental cost-reduction and/or revenue-generation efforts.Contributing to or leading departmental quality improvement efforts as approved by the manager.Serving as a preceptor to pharmacy residents and pharmacy students.Attending required hospital-wide orientations, meetings, and in-services.Demonstrating a commitment to flexible work scheduling when necessary to ensure patient care.Being cross-trained to work and provide services at all outpatient pharmacy locations.Qualifications include:To perform this job successfully, an individual must be able to perform each essential duty satisfactorily.
