Full Time Registered Nurse Case Manager, Level 2 Anticipated Territory: Northwest Sacramento including Natomas, Arden/Arden‑Arcade, North Highlands, Del Paso Heights and Elverta. What you’ll do as a Registered Nurse (RN), Level 2 Home Health Case Manager Utilize your outstanding clinical skills as you create, implement and execute the overall plan of care to promote wellness and prevent read‑hospitalization for patients in their homes. Provide oversight of 2 Nurses responsible for executing your care plan. Conduct comprehensive patient assessments such as Start of Cares (SOCs), evaluations, resumption of care and discharge planning. Perform a full assessment of the patient & family upon initial visit. Use assessment data to develop an advanced plan of care including rehabilitation. Work one‑on‑one with patients and family to achieve improvement and/or stability. Case‑manage patients’ care episode with interdisciplinary clinical team. Prepare clinical notes, orders and charting in a compliant timeframe. Collaborate with physicians and agency staff to deliver the best care possible. Qualifications and Requirements One year of Home Health Case Management experience preferred; minimum one year of Nursing experience required, two+ years preferred. Experience supervising, mentoring nurses highly preferred. Current, unrestricted CA RN license. Current BLS/CPR certification. Valid CA driver’s license. High level of clinical knowledge in home health care. Experience with high acuity patients a plus. Experience working with an interdisciplinary team required. Case Management fundamentals required. Compensation Annual salary range: $140,000 – $160,000, plus up to $20,000 annual bonus paid quarterly.
