Big Leap Family Guidance Centers is seeking a dedicated and compassionate Licensed Clinical Social Worker (LCSW) to join our remote team, specifically serving the dynamic needs of our Hospice Center in Groton, Connecticut. In this full-time role, you will be instrumental in providing critical mental health support to individuals during some of the most challenging times in their lives. This position offers a competitive salary ranging from USD 80,000 to 100,000 annually, along with a sign-on bonus.

While the majority of your work will be conducted virtually, occasional in-person meetings at our Hospice Center in Groton may be required for collaboration and team-building purposes. • Conduct thorough psychosocial assessments to understand clients' mental health statuses, strengths, and resources.

• Design and implement individualized treatment plans tailored to clients’ needs, setting specific therapeutic goals.

• Deliver individual, family, and group therapy utilizing evidence-based modalities such as cognitive-behavioral therapy (CBT), solution-focused therapy, and trauma-informed approaches.

• Collaborate closely with other healthcare professionals, including psychiatrists and psychologists, to provide coordinated and comprehensive care.
\n
• Facilitate group therapy sessions and offer psychoeducation classes that empower clients through shared experiences.
\n
• Provide crisis intervention services, including risk assessments and safety planning when required.
\n
• Maintain accurate and timely clinical documentation in compliance with ethical guidelines and legal standards.

Your role will focus primarily on clients facing end-of-life issues and their families, delivering sensitive and individualized mental health support. You will work closely with patients experiencing grief, anxiety, and depression related to terminal illness. Your ability to develop trust and rapport with diverse clients will be essential in creating a therapeutic environment conducive to healing.

• A Master’s degree in Social Work from an accredited institution is required.

• Active state licensure as a Licensed Clinical Social Worker (LCSW).

• A demonstrated commitment to continuous professional development in mental health practices.

• A minimum of two years of experience working in a mental health or outpatient setting, with a focus on hospice care being highly desirable.

• Expert knowledge of evidence-based therapeutic approaches and a strong understanding of psychosocial dynamics related to end-of-life care.

• Excellent communication and interpersonal skills, enabling effective patient interaction and collaboration with multidisciplinary teams.
