Sun River Health is currently seeking a full-time Psychiatric Nurse Practitioner to join our team supporting the Inwood site in Bronx, NY.Position SummaryA Psychiatric Nurse Practitioner provides psychiatric clinical services for patients. Performs job duties in compliance with systems and job specific standards.Job Type: Full-TimeSalary Range: $159,724.50 - $172,614 per yearSign-On Bonus: $3,500 Sign-On Bonus for Eligible External Applicants – Terms and Conditions Apply.Essential Duties and ResponsibilitiesConsult with MD/Psychiatrist/LCSW on patients via eCW, telephone encounter, and/or with patient visitsConsult with MD/Psychiatrist on medication management, via eCW, telephone encounter, and/or visit with patient as needed to address BH concernsProvide clinical care (medication management/psych evaluations) for patients to support efforts to achieve care plan goals, in support of ongoing care by MD/Psychiatrist/LCSW, with referral back to ongoing care with clinical teamParticipate in case conferencing and high-risk meetings with LCSW/MD/Psychiatrist and care teamsApproval of treatment plans for patients in BH care at Sun RiverParticipate in efforts to support Sun River's primary care and BH integration effortsEnsure appropriate transitions of care with external providers, PCP, hospitals, etc.Respond to Sun River clinical teams for crisis interventionInterviews, diagnoses and evaluates patients referred for psychiatric assessmentParticipates as a team member in conferences concerned with diagnosis and treatment planningWrites reports and documentation as requiredConsults with other mental health and medical provider staff regarding treatment of patientsPrescribes psychotropic medication as needed and monitors its effectivenessProvides in-service training to professional and paraprofessional staffReviews case records to monitor quality of careMust demonstrate knowledge and skill necessary to provide care appropriate to the age of the patients servedMust demonstrate knowledge of the principles of growth and development over the life spanMust be able to identify and categorize each patient's age–specific grouping of needs, such as those for infant, adolescent, or geriatric patientsMust be familiar with the process of diagnosing and treating patients with substance use disorder and make appropriate referrals when necessaryEssential Job QualificationsMinimum Education Requirement: Master's DegreeMinimum Licensure: Nurse Practitioner in Psychiatry and CPR/BLSRequired Skills & Knowledge: - Knowledge of diagnostic and treatment procedures that nurse practitioners are trained to do and are necessary for practicing in health care centers. - Competence in delivery of health care to patients (including preventative health education) - Ability to collaborate with other members of the Health Center in order to maintain high quality standards and achieve client satisfaction. - Bilingual in both English and Spanish (orally and written) is preferredAbout Sun River HealthSun River Health has a rich history of providing quality, affordable health care to communities in need.
