Competitive pay & benefits – Including medical, dental, vision, 401(k), generous PTO, and more!Your Role as a Support CoordinatorAs a Support Coordinator, you’ll work directly with Medicaid participants in self-directed care programs. Your role is to educate, guide, and support them in managing their care plans, hiring direct care providers, and ensuring their home environment is safe.You'll be out in the field meeting participants in their homes for at least 60% of your work hours, ensuring they have the resources they need to remain independent.
