Field-Based Care Manager — Behavioral Health & Recovery

MetroPlus is seeking a Health and Recovery Plan (HARP) Care Manager to develop and communicate plans of care for members. This role requires assessing complex health needs and performing case management activities in the field. The ideal candidate will have a Master's Degree and 3-5 years of Behavioral Health experience.
