Full-Time Orthopedic Physical Therapist - Home Care Setting

Position Overview

Motion and Health Rehabilitation Center is seeking a dedicated and skilled Physical Therapist to join our team in Monroe Township, New Jersey. As a key member of our home care unit, you will play a vital role in helping patients recover from orthopedic conditions through personalized therapy plans and hands-on care.

Work Environment

This full-time role is performed onsite, primarily in patients' homes, where you will bring rehabilitation services directly to those in need. Our environment fosters collaboration among our staff, including physicians, occupational therapists, and nursing professionals, ensuring that you have the support needed to provide outstanding patient care. The typical work schedule is Monday through Friday, from 9:00 AM to 5:00 PM, allowing for a balanced professional and personal life.

Key Responsibilities

• Assess and evaluate patients' physical conditions and needs to establish effective rehabilitation strategies.
• Develop and implement individualized treatment plans tailored to the unique requirements of each patient.
• Conduct therapy sessions focused on orthopedic rehabilitation, utilizing manual therapy techniques, therapeutic exercises, and education to guide patients toward optimal recovery.
• Maintain accurate and up-to-date patient records using CureMD EMR, documenting progress and treatment modifications as needed.
• Collaborate with a multidisciplinary team, including MDs, OTs, DPTs, NPs, and PAs, to ensure comprehensive care and achieve the best possible patient outcomes.

Patient Population / Client Focus

The primary focus of your care will be on adult patients, typically aged 40-65, who are experiencing orthopedic challenges. These may include post-surgical rehabilitation, injury recovery, chronic pain management, and pre/post-operative education. Your ability to communicate effectively and compassionately is crucial in engaging patients and motivating them through their recovery journey.

Qualifications

• A Bachelor's Degree in Physical Therapy from an accredited program is required.
• An active New Jersey Physical Therapist license or eligibility to obtain one is necessary.
• Strong clinical skills, with an emphasis on manual therapy and evidence-based practices.
• Exceptional interpersonal skills to foster rapport with patients and facilitate their recovery.

Preferred Experience

• A minimum of two years of experience in a related outpatient setting is preferred, though motivated new graduates are encouraged to apply.
• Experience with elderly populations and familiarity with home care settings is an advantage.

Compensation and Benefits

We are pleased to offer a competitive salary range of USD 110,000 – 125,000 per year, commensurate with experience and qualifications. To support your overall well-being, we provide:
