Clinical Care CoordinatorPay Range: $36/hr + Bonus Structure *Pay varies depending upon credentials/educationCredentials: Physicians, psychologists, LICSW, LGSW, APRN, RN, LISW, LPC, LMFT, and LGPC requiredJob SummaryThe Clinical Care Coordinator service adopts a "whole-person" approach to address the client's needs related to physical health, behavioral health, and social determinants of health. CCC involves the coordination of care between the behavioral health clinician and the clinical personnel of an external provider (e.g., primary care, another behavioral health provider, hospital). Clinical Care Coordination occurs when the practitioner via direct face-to-face contact, video-conferencing, or telephone communicated treatment needs, assessments, and treatment information to external healthcare providers and facilitated appropriate linkages with other healthcare professionals, including transitions into or from a higher level of care or institutional settings. CCC also includes treatment planning and plan of care implementation activities that are separate from the diagnostic assessment service when the clinician and client are meeting face to face or via video conferencing. The Clinical Care Coordinator is responsible for ensuring that the client is at the appropriate LOC. If the clients fail to make progress or have met all of their treatment goals, it is the Clinical Care Coordinators' responsibility to ensure timely assessment and transfer to a more appropriate LOC.Summary of Essential Job FunctionsThe key responsibilities of the Clinical Care Coordinator include but not limited to:Facilitate outreach to establish referral providers and participate in outreach eventsFacilitate intake admissions and reassessments as scheduledBoard approved and active licensure in the District of Columbia (assigned credentials outlined in Job Description)Prompt and regular attendance at an assigned work location.Ability to complete [or oversee the completion of] assigned tasks/projects in a timely manner.Carry a caseloadDetermine the frequency of ongoing Diagnostic Assessment and Plan of Care servicesCollect available social, educational, economic, and vocational information to be used as part of the assessment processImplement or revise patient treatment plans with the patient as required per state regulation and record a review of the patient's progress according to BHG policyDevelop within (7) days of entry or as required by state guidelines, whichever is more stringent, a formal treatment plan documenting patient goals and update utilizing the same guidelinesMaintain patient records that are complete, comprehensive, and well organizedSubmit records for periodic review as requested by the supervisory teamDocument all patient contacts. If other team members provide services to the patient, the assigned counselor is responsible to see that such contacts are documented in the patient's chartMake appropriate referrals through case-management services and perform necessary follow-upAssist the Clinical Director in implementing new programs into patient treatmentAdvise the Clinical Director of problems encountered with caseload management and schedulesAttend weekly treatment team meetings. Be prepared to discuss his/her assigned individual patient caseloadHold a basic understanding of alcohol/drug abuse and addiction. Additionally, keep up- dated on current drug abuse and treatment information, especially current trends in Opioid Maintenance TreatmentProvide intake/readmission services, including several weeks of supportive follow-up counseling for new patients until their assignment to a primary counselorProvide, monitor, and document follow-up calls/contact to all discharging patients for at least 30 daysProvide crisis intervention/support services to patients assigned to other counselors when their primary counselor is unavailableProvide inter-agency reports/documentation required for third party reimbursement, criminal justice proceedings, supplemental income assessments, emergency services, and other specialized treatment needs of patients as applicableCoordinate care with other community providers as neededOther duties as assigned by supervisory teamRegulatoryEnsure compliance with all federal and state regulatory bodies, as well as accrediting agenciesComplies with all federal, state, and local regulatory agency requirementsComplies with all accrediting agenciesMeet BHG attendance standards and be present and ready for work at the beginning of their established work scheduleReport any work-related injury or illness, workplace hazards, or security issues immediately to the supervisorMarketing and OutreachParticipate in community and public relations activities as assigned.Professional DevelopmentResponsible for the achievement of assigned specific annual goals and objectivesDemonstrates the belief that addiction is a brain disease, not a moral failingDemonstrates hope, respect, and caring in all interactions with patients and fellow Team MembersEstablishes and maintains positive relationships in the workplaceCan work independently and under pressure while handling multiple tasks simultaneouslyMakes decisions and uses good judgment with confidential and sensitive issuesDeals appropriately with others in stressful or other undesirable situationsTrainingParticipate in and provide in-service trainings as required by federal, state, local, and accrediting agenciesAttend conferences, meetings and training programs as directedParticipate in and/or schedule and attend regular in-service trainingsUp to date with the latest softwareOtherDemonstrated commitment to valuing diversity and contributing to an inclusive working and learning environmentMinimum Requirements and QualificationsThe Clinical Care Coordinator must hold one or multiple of the following licensures: Physicians, psychologists, LICSW, LGSW, APRN, RN, LISW, LPC, LMFT, and LGPC. Also, must have the appropriate experience for the role.
