Key Responsibilities\n

• Conduct thorough psychosocial assessments by interviewing patients and their families, observing behaviors, and reviewing medical records to evaluate emotional, social, and environmental factors that may influence recovery.

• Deliver counseling services to patients, helping them address adjustment issues, emotional challenges, and provide strategies for coping with the changes they are facing in their lives.

• Collaborate with the rehabilitation team to formulate individualized treatment plans that incorporate psychosocial assessments, ensuring that every aspect of a patient’s recovery is considered.

• Help connect patients with community resources, such as support groups or vocational rehabilitation services, that can assist them in their recovery process and promote autonomy.

• Educate patients and their families about available resources and services that may facilitate their ongoing care after rehabilitation, contributing to a smoother transition back to their home or community.

• Participate proactively in discharge planning, ensuring that all necessary resources and support systems are established for patients as they move towards independence.

Patient Population / Client Focus\n

In this role, you will primarily work with a diverse patient base encountering unique challenges related to recovery from strokes, spinal cord injuries, orthopedic surgeries, and various neurological disorders. Your role will be crucial in not only addressing their immediate emotional and social needs but also empowering them to regain confidence and functionality in their daily lives.

Qualifications\n

• A Master’s degree in Social Work from an accredited program.

• Possession of an active state license as a Licensed Clinical Social Worker (LCSW) is mandatory.

Compensation and Benefits\n
